FORMS ARE IN PDF FORMAT AND ARE NOT DESIGNED TO TYPE ON-LINE. YOU MUST HAVE ADOBE READER ON YOUR COMPUTER TO OPEN AND DOWNLOAD FORMS FOR COMPLETION. PLEASE PRINT OUT AND WRITE LEGIBLY IN BLUE OR BLACK INK.

• High Risk Activity Permission (2 pages; sign both pages)

• Disciplinary Policy

• Current copy of immunization certificate, signed/dated by physician (must still be current during camp)

For the safety and well being of all individuals in the camp,we are unable to accept campers who are not immunized for any reason.
