Current copy of immunization certificate, signed/dated by physician (must still be current during camp) [For the safety and well being of all individuals present in camp, we are unable to accept campers who are not immunized for any reason].
It is important that all forms and payment are sent at time of registration. Any missing forms or missing fees will result in your daughter being placed on the waiting list until missing forms and/or payment are received, which could result in your daughter not getting into day camp if the unit fills up with other registrations.
